NOTES to New Patients: If you have had a full mouth series of x-rays taken within the past 3 years, it is your responsibility to either bring them to your appointment or have them mailed directly to us. If we do not have them by your appointment date we will need to take a new set.

Also, we ask that the patient pay for the first visit at the time of service. We will have insurance reimburse you directory for this visit. Thereafter, we will file insurance on your behalf.
